My girlfriend is 200+ lbs at 5’5″ and has asked me to design a weight loss program and diet for her but she really wants to start off with a 7 day smoothie only fast type thing. So I came up with a smoothie that has mixed frozen fruit, a banana, skim milk, some kale, and a scoop of vanilla whey protein powder. Drinking 4 of these a day ends up at about 1600 calories, with 214g of carbs, 143g of protein, and 15g of fat. It also ends up with 28g of fiber.
Now from what I’ve read, I know carbs tend to be the main problem for women with PCOS and insulin resistance. But, I’ve also read that consuming that fiber and protein at the same time as the sugars/ carbs really slows down the absorption of the carbs significantly enough for her insulin to not really spike all that much.
So my question is, Is this a safe thing for her to do with her PCOS? I want to help her, and I plan on dieting with her the whole time and changing my habits too to help her in the long run, but I know her needs are different and I want to do this safely for her. Anyone with any experience or advice would be greatly appreciated. Thank you.

I would do green smoothies, but without the whey or milk. It’s very irritating to the systems, and creates a lot of mucous in the tissues…in particular, it irritates the intestines, which need to be in optimal shape to absorb the nutrients from the good food you give her. No milk, and no soy. The kale is loaded with amino acids which will be quickly made into the proteins her body needs after digestion. You will also be doing her a favor if you switch up the green and the fruit with each smoothie. Green smoothies should be varied and simple so that they are easily taken into the body. Too many ingredients in them totally defeats the cleansing effect they are meant to have. On the long term, she should consider changing over to the “paleolithic diet” …a biologically appropriate whole foods dietary overhaul.

It sounds pretty balanced with C, B, D & K vitamins, provided the kale has enough iron (women have higher iron needs than men) and you might want to consider a fortified soy milk for essential omega 3 oils.
To be honest I don’t think drinking one drink only is the right way, I think you want to replace one or two meals a day (say, breakfast & lunch) and gradually start towards eating healthy low-fat/carb, high-fibre/protein meals.
What you say about insulin resistance sounds right to me. PCOS is a syndrome, a set of symptoms which shouldn’t interfere with the diet itself but maybe advice from a dietitian would be good anyway? They have good ideas for long-term change and getting there.
